Abusive language or behavior toward our staff will not be tolerated.
Patient Paperwork
To provide the best care, we require registration paperwork for every patient to ensure we have accurate information on file.
Please click the Patient Paperwork link to the left to complete your forms online.
When finished, simply sign and click Submit—your paperwork is sent directly to us.
There’s no need to print or email the forms.
Remember to bring your driver’s license/photo ID and your insurance card(s) to your appointment.
Appointment Reminders & No-Show Policy
We send appointment reminders by email only, so please provide a valid email address to receive them.
You can confirm your appointment directly through the email reminder.
(We do not make phone reminder calls.)
If you need to cancel or reschedule, please notify us at least 24 hours in advance.
Missed appointments without notice prevent other patients from receiving needed care.
A $50.00 no-show fee will be charged for any appointment not canceled in advance.
This fee is not covered by insurance.
